| Introduction |

Primary health care is probably the most frequently mentioned pathway of health care reform in the world today. Advances in genomic sciences, nanotechnology, and information technology are generating the knowledge and tools required to implement primary health care. We are clearly at the dawn of a new era in health care delivery, where transformational changes are bound to occur in the patient-doctor relationship as well as in the inter-professional relationships on the health care team. Integrated, well coordinated health care ¡V akin to the way a symphony orchestra makes memorable tunes ¡V will be the universal standard of health care.
This conference on ¡§Primary Health Care - A Symphonic Approach,¡¨ is organized by the School of Pharmacy to celebrate its 15 years of service to Hong Kong and to showcase the opportunities on the horizon that will diversify and expand the role of pharmacists in Hong Kong in the next 15 years. The conference is comprehensive in scope. A distinguished panel of pioneers and leaders in modern health care delivery has been appointed to share experience and views in primary health care, success stories of primary health care in disease prevention, chronic disease management, and formulation of health care policy. The central importance of open communication on the health care team is underscored by the active participation of the Nethersole School of Nursing and the School of Public Health, together with the School of Pharmacy, in designing an outstanding program for the conference. The conference concludes with a round table discussion by health care leaders, policy makers, and health care professionals that will lead to a position paper. It is my hope that this conference will accelerate the pace of transformational changes that Hong Kong needs to continuously elevate the quality of cost-effective health care.
